You can try to turn off the included CSS under “Survey Options” select “no” for “Use included CSS” – make sure to save. That will allow your theme’s CSS to display form elements however it displays form elements but if the inputs are still too small, you may need to contact your theme author for help with that or you may need to create some custom CSS to display it to your liking.
