Also getting 404 errors when attempting uploads

The upload box is not rendering correctly in firefox and chrome (just a white box, does not show file path when selecting file, does render fine in IE. However all three browsers report a 404 error when attempting to execute upload.
Both my sites are doing it.
Please help
