ALL comments are published, in spite of certain settings

I have not checked the box in Settings/Discussion for “Comment must be manually approved,” but I have checked “Comment author must have a previously approved comment.” But, ALL comments, even first-time comments from site visitors, are being published without moderation, and, even if the person’s IP address or name has been entered in the Comment Moderation block that is supposed to hold the comment in the moderation queue.
