All cached pages via PHP

Hi,
I’m using WPFC for quite long time. Now I noticed that on one very large site all cached versions have note <!– via PHP –>. Only change I made was to allow caching of pages with query strings.

htaccess is working, mod_rewrite, all as always but now it’s served via PHP. What can cause that?
