    I had the same issue. Make sure you have jQuery enable on the page where you put your form.

    Usually this happens when jQuery is not enqueued, the wrong version is enqueued, or your theme or another plugin are causing a JavaScript error on your site. Do you see the same behavior on a default theme, or with other plugins disabled?
