Ajax issue

Whenever I try to submit the form on https://www.jaysmetal.com/products/duraclad I get this error

issue it will just create the pdf and send email notification but just that pressing submit button will not redirect to thank you for submission page

I isolate the issue by deactivate this plug in and the error goes away.
