  • Hello @bahl2,
    This form doesn’t come from WP Maintenance Mode plugin for sure.
    Most likely you have another third party plugin installed that is outputting it.

    I would recommend to deactivate all your plugins one by one and you will find for sure which one is causing this.

    @bahl2 If nothing happens after you deactivate each plugin, I think you should check all WordPress files. You’re using a pretty old WordPress version (v5.2.7), and someone might have hacked your website.
