Advice on creating a form

    I want to create a form that has the following properties.
    5 users have access to the form and need the ability to add text then save the form.
    The next user will see the previous users comments, add there own then save the form.
    When all five had added there comments I need an option to email the completed form.
    What is the best way to achieve this. I have it working via Google Docs but would like to keep it within my wordpress site if possible any advice good plugins etc welcome
