adress in input field?

Hi there.

first of all: Thanks a lot for this plugin. It helps me a lot.

But i got a little question. I want my customers to put an adress in extra input fields instead of just the email and the name. I tried to change your code on your site and add it with pluginception, but he always tells me “invalid street” because i just changed the phone to street.

Could you tell me how to get adress fields?
Would be nice. I dont find the sites in edit mode where to put the fields in..
