Admin menu is showing up on the front end

I have installed wordpress over 200 times and I have never ran into this issue before.

This is a completely fresh install of wordpress 3.3.1

I have installed 3.3.1 on my server for 5 other sites with no issue, so I don’t know why this is happening.

I have installed, uninstalled, deleted the account from the server, recreated the account, reinstalled, uninstalled. Usually wordpress takes me about 30 minutes to setup, but I have spent 6 hours trying to figure out what is going on. Below is a links to a screenshot of the problem.
