Address page shows blank

Hello and thanks for the plugin, it looks just like what I need, but I’m unable to make it work. When I activate it, the address page on my account page shows a blank page. I can see the page loading, and see the addresses for a moment but then they disappear as if some other content overlaps it. I tried disabling all other plugins but the issue persists. I’m using shopkeeper theme. Could you please help me out, as I need this functionality very badly. Thanks in advance!
