Address fields missing in Square payment page

> The standard checkout flow pages are working properly on my site, but the square plugin does not ask for the customer’s address and country if they use a customer payment page that’s generated via an invoice I’ve created for them.
How do I add the address and country input fields for this checkout flow?

Assuming that your customer payment page is overwriting the default WooCommerce checkout template you will probably have to add these by adding the required code to your template file.

Where can we see an example of the this payment page online?
