Additional Form Fields & Live form Issue

I’m having two issues with this plugin and hopefully someone can help me.

1. Using the opt-in form creator in the plugin (NOT the form creator in the actual IS app), I cannot figure out how to add additional form fields. The most I can do is First Name + Last Name + Email. I’d also like to add State and possibly Phone.

2. When I create an Opt-in form in the plugin, the Image, Title, and Description aren’t visible when I make the form live. The form looks good in Preview but not in Live. See screenshot here: https://i.imgur.com/PzUXm1e.png
