It is likely that you have a javascript error on the page. You will need to look into the browser console to see the error. When it is removed, the page should be working fine.

I don’t see any errors. It looks like there are just warnings about some scripts: Scripts that have a dependency on [wc-settings, wc-blocks-checkout] must be loaded in the footer, klaviyo-klaviyo-checkout-block-editor-script was registered to load in the header, but has been switched to load in the footer instead.

I have a test site setup and disabled all plugins. I’m still not able to get the form to work and create a new registry: https://mybncsite.com/christopher-collection-new/wedding-registry/new/

If you have disabled all plugins, have you also changed the theme, just to rule out a conflict from that as well? If you have done this and the problem still remains I will need to look into it directly on your test site. Then send me login details to the site through the plugin support system.

I changed the theme to default wordpress and looks like it’s working now. How would I go about troubleshooting theme conflicts with plugins?

You probably need some developer knowledge to do the troubleshooting, but I suspect that it is a javascript problem. The theme has a javascript error that is preventing the plugin javascript from working.
