Adding new sites = sudden bugginess

Hi there. Last week, and again today (just updated us to 3.8), I’ve been encountering the following weirdness when adding new sites to our multisite installation:
1) Mysteriously, 4 days ago, after adding a new site to our multisite installation and receiving the normal confirmation email:
–the new site did not appear listed from the My Sites menu *or* from Network Admin > Sites
–clicking on the link to the site from the link provided in the automated confirmation email turned up a 404. Truly, this site doesn’t seem to exist. Except there was an email generated confirming that adding it did happen.
2) Today, after updating to WP 3.8, I tried creating a new variation of this site, with a slightly different name and subdirectory name.
–No confirmation email. But the site now seems to exist, at https://…/newsitename. And I’m able to admin its settings from there.
–The site is not listed from the My Sites menu.
–The site *is* listed from Network Admin > Sites.
What could be going on here? Our .htaccess and wp_config files have not changed recently, and up to 3 weeks ago, all was normal adding new sites (we have 6 in addition to the root) to this installation.
Is anyone else finding this happening? Weirdly–and possibly unrelated–I just today discovered that a user I had set up months ago is not showing up in either the network or site user list I’d added her to. She had never confirmed her email, but she isn’t listed anywhere and so I cannot reset her welcome msg. and also can’t re-add her using the same username or email. So she’s in the system but just not showing up. I’m not expecting help for this issue here on this post, but am just posting this here in case both issues are indicative of a data issue or other discrepancy others have heard of.
