Adding name info into the form

    Hi,
    Awesome plugin! This saves us so much time in gathering and collating the results.

    One thing I was wondering…

    It is possible to add a name field to the form so that our admin knows who it is coming from? The only way we can tell at the moment is by recognizing their email address, which may be difficult to keep track of once we go live with it.

    NVM – figured it out:
    Inserted the following at line 346 and 347 of the spiritual-gifts-survey.php file.
    <p style=’text-align: center’>Your Name:
    <input type=’text’ name=’name’ />

    Glad to see you found a solution!

    I added a “name” field into version 0.9.9, as well as fixing a sorting issue with scores below 10%, so I encourage you to update to the latest version.
