• I’m wondering if it’s possible to create a radio button form and utilize images instead of text as the options. For example, I rent photo booths, and instead of using radio buttons saying “red,” “gold,” “purple,” etc. for the curtain options, having small photographs of the curtains.

    Thanks so much in advance!

  • I would love to see this too! I’ve been Googling for the last hour trying to find something similar but no luck. A plugin would be cool for this.

This can be done really easily using the separate values in the pro version. Please post in our help desk if you’d like more information.

I took a look at the pro demos and I’m not seeing what I’m looking for. I have a client that does granite counter-tops and I’d like to create a form that allows a customer to pick out options for their counter. These options need to be in the form of images like

    • Edges
    • Finish
    • Pattern
    • Color

At the end, there would be an option to submit all this data to the website. Is this possible? Can you show me a demo that is similar? I’d be happy to pay for the pro version if this is the case.

    Hi Shaye,

I put a very simple demo of this up here:

Let me know if you have any questions about it.

Thanks Jamie. I ended up using Gravity forms which I already own. Just didn’t realize it was capable of doing this too. If it doesn’t work like I need it to, I’ll definitely take a look again at your forms.
