Having looked at the Post structure and the custom fields, it looks like it should be possible to add an image to a post that had been submitted by User Submitted Posts. So I tried adding an image (using one from another post) by adding a custom field user_submit_image with the image information as the value.
    It did not work.
Any idea why not and how I might be able to do this should I really need to?

Trying some other options I find that deleting a custom field user_submit_image does not remove the image and changing the filename does not change the image so even more puzzled now!

I know exactly what you are talking about, but unfortunately I am unable to figure out why it happens (or doesn’t happen in this case). There is a workaround, however but it is sorta clunky:

    1) Add the new image to the post using the “Add Media” button
    2) Then after saving the changes, you can add the user_submit_image custom field
    3) Save changes again

The trick here is using the Add Media button to get WP to recognize that the image should be attached to the post. Just adding the custom field by itself is not enough.

Let me know if any further questions or feedback, glad to help anytime.

For your second question, which you posted before I was able to type my response, the same idea applies: changing/removing user_submit_image custom field is not enough to tell WP that the image should no longer be attached to the post. If you delete the image via the Media Library it will be deleted for good. Then you can delete the custom field, etc.
