Adding Custom form to Product Page

I would like to add a form to some of my product pages — I am selling personalized items and need to have the customer add the personalization information. When I click on Add Form I can only choose from contact forms or subscribe forms.
Can anyone recommend a plugin that would allow me to do this? Thanks
