I hope you already found a solution. If not you can easily modify the country drop-down plugin to do what you need.

Hide the phone in the short code or widget’s options.
To hide the phone in the short code set phone equal to Zero
[easy_sign_up title="Your Title Here" phone="0"]

How To Use:
Cut and paste the code block into a text file – name that text file: esu_dropdown.php and save
Put that new php file into a folder called esu_dropdown
The zip the folder
