Adding checkbox to WP login form, to accept terms before logging in

Hi,

I am trying to add a checkbox to the WordPress login page, where the user has to check the box before logging in. This is to ensure they read a terms/disclaimer before logging in. Does anyone have any plugins or code I can use to do so?
