Adding attachment button

Hello,
I would like to create a CV submission form in my website. I’ve tried a couple of plugins but they are not exactly what I want so I’ve decided to create custom made page by myself. The problem is I don’t know how to create a button that allows the candidates to attach their CV’s + Picture and send it to my email address. I will be really thankful if anyone can help me with this specific problem. Or at least suggest me a plugin to do so.
