Adding a survey conditionally to the woocommerce checkout process

    Hello,

    I’m a newbie to WordPress, literally start to know about it now.
    I’m thinking of changing my vanilla JS-coded site to WordPress for easier administration.
    So I am searching for the plugins that can replicate the features that the site has.

    And now, I am stuck on finding a solution to adopting a survey(a short form) middle of the checkout process.

    The conditions are:

    1. The survey is located in the middle of the checkout
    2. The survey content is dynamic depending on your item
    3. Non-coder should be able to create and modify the survey and the condition.
    4. Prefer a Free or very-low-cost plugin
