Adding a form before download a document with WP-Downloadmanager

Hello:
I’m using WP-Downloadmanager to manage documents and files.
Now, I need to use a form before to download the document. A simple form with 2 or 3 fields: name, email, and one button selection.
Do you know how can do it?
