Adding 1 media creates multiple attachment ID

    Hi,

    I’m running Worpress 4.0 and when I add an image through the media menu, it creates a huge amount of attachment id. Adding 1 image creates hundreds of new images (attachment id’s). I tried to deactivate all plugins or put default theme but it still does it.

    Any idea?
