Add some new fields on button click

Hi

This is really a great plugin.

I am using this plugin one one of my site. But now I have a condition when I click on add more filed button then some extra fields like.
on page load repeatable fields form show only name and last name but when I click on add more button then it again add name, last name and phone number.
Is this possible.

Or

Let me know can we make repeatable fields hide and remove on page load and repeatable fields only add on form on ad more fields button click
