add same info to every record

I’m setting up a Contact 7 intake form (using CF& Skins) with your excellent CFDB so that an admin assistant can log into admin and retrieve individual records.

She prints out each record and faxes it to medical providers. She would like to have her agency name and contact info on the top of each record (like a header in Word). She might also want a logo image there.

How would I do this?
