Add radio buttons to event submissions form

How do you modify the event submissions form? I can find the template file but how do I add 3 radio buttons that I can pass to the email that goes to the person who approves the events?

I was thinking this would be part of the custom booking forms offered with the Pro plugin but can’t find it.
