Add Media — no "insert into post" option

I have searched the forums and found otto’s very detailed how to add media instructions and followed them before and it worked (2 weeks ago) and now today am trying to follow them again but after selecting upload the pdf goes straight to my list of media with no option to “file url” or “insert into post”. I’m brand new to this …
