“Add Media” button not working

I have discovered an issue with my site where the “Add Media” button doesn’t add anything to the post. It opens the “Insert Media” window and lets me do all the normal things like uploading, editing titles, captions, alt text etc… But when I hit the “Insert into post” button, the window closes but no images are actually added.
