Add Media button / Hyperlink button and html editor not working

Not able to add hyperlinks, add media, or use toggle between visual and text editors…
Not sure what caused the problem… currently using customized theme but I tried reverting back to default themes (including 2015) with no fix. Also disabled all plugins and deleted most … still not working. Any ideas?
