Add image title/alt/description without using "Manage Gallery"

I’ve searched to no avail…
I’ve created a form that my users fill out when they upload images. I’d like to be able to take information from this form and insert it into the image metadata attributes without having to go into “Gallery | Manage Gallery” page.
Any help appreciated!
