Add Form instead of add to cart button

Hello,

I am using this plugin for bulk order process. But the problem is that instead of add to cart button i want to show a simple form (like name, email etc). when we click on submit button of the form all information like (product title, quantity) which is fill on bulk order form. These information will send to mail id.

Is this possible?
