Add date of birth to invoice

Hi there, my client wants to add date of birth to invoice for pregnant women to declare the invoice to the health insurance. Is that possible?

And how about a different productname? We pretend not to be a pregnancy course. But in fact for pregnant women it is a sort of pregnancy course. For the health insurance they need the productname to be “pregnancy course” instead of “montly or annualy membership”. That is how we sell our course. Is that possible in some way?
