Add custom fields (e.g. "title", "affiliation", etc)

Hello!

This plugin looks quite good, but I’m looking for a way to do two things:

1) By default, it only display the initial of any signatory’s surname: how can I have it include the full surname?

2) I would like to add custom fields for “Title” (e.g. Mr., Dr. etc) and “Institutional Affiliation” (optional). Can it be done? If so, how?
