Add additional form information to product

Hi all!

I’m trying to figure out if this is even possible. I have a site with business cards as one of the products. I would like to add fields for the customer to fill out for their card information.

Name_______ Phone______ etc.

I also would like to add an additional choice.
Would you like your photo added? Yes or No and the ability to upload either during checkout or prior to purchase (within this form).

I truly hope I make sense, and if someone could point me in the direction. I’ve searched, but it’s fairly obvious it’s either not possible or I’m failing to come up with the correct verbiage. Any help would be much appreciated!
