Add a form field value in shortcode

I add a form built in your plugin on a wordpress page using another plugin.
I want to add a value to a hidden field to show the page the form is loaded from.
The referrer data is often empty and the data showing what page the from is submitted from only shows the ajax function that loaded it. So I’m wondering if I can add a form field value in the shortcode.
Example: [accua-form fid=”5″ myfield=value]

The other plugin I use to load your form has the ability to add the permalink when it loads the form, I just need to know if your forms can read the extra code in the shortcode to add the value.
