Ad Buttons Not Being Created

I go to “Ad Buttons”, enter image, link, text and ad position and click Create Ad Button and I get a message that the button has been created successfully, but I don’t see the button listed below and there are no rows in the ad button table in my database.
So it appears that buttons are not getting saved.
