Actions after submit not working

  • Hi There,

    I have recently taken on an existing website with a client at the above domain name.

    When running testing for the contact form the website would crash after submitting the form, as well as this the datepicker field does not seem to be functional.

    I have a screenshot of the errors seen in the inspection panel which may help

    Please can someone assist here?
