Acknowledging the reading of documents

Hi,

Im looking to implement a method of recording who has read certain documents within my wordpress page. I currently have access controlled via logon so what I now want to do is offer some sort of “Ive read this document” acknowledgement by either a tick box or acceptance button.

Does anyone know how I can achieve this please?
