Acceptance checkbox not display

Hello,
at the end of my form, i’ve added an Acceptance checkbox. I can see it when i’m logged in the WP admin. But if i access to this page on an other navigator (not logged) or with private window on the same navigator, the Acceptance checkbox and its text are not displayed.
Any reason why ?

the same problem.. even deactivating all the plugins…
no radio , no checkbox…
