A weird post tag issue, any help or guidance is appreciated

Alright, every single time I add tags to my posts, everything appears as it should be, however when I go to the back end section, sometimes the tags are posted as

Name Slug Posts
Parenting parenting 9

that is all right, but sometimes, randomly, it becomes

Name Slug Posts
Pregnancy & Parenting pregnancy-parenting2 1
Pregnancy & Parenting pregnancy-parenting3 1
Pregnancy & Parenting pregnancy-parenting4 1
