A plugin/solution for marketing data capture/form

I have a client who wants to send his advertising clients lists of any website users who indicate an interest in their products, so a bit like a marketing data opt-in form.

So basically it’s a form with Company A, B, C and more listed with a checkbox for site users to tick if they want to share their details, which are then sent on to the advertisers. Now I can achieve all that with any number of form plugins and conditional logic.

But the client has a few other considerations.

(a) The companies will change from month to month so there must be some way of inputting their data and then easily adding or deleting them to the form – but so the details are sent automatically to the companies.

(b) To complicate it further there’s a step in between where the client wants to approve each request first, then send them on as a batch to the selected companies.

Some of it I can achieve with GravityForms, etc, but not all. Does anyone know of a sort of marketing data management plugin that might achieve this?
