A Plugin To Gether Contact Information, Other Info

I’m in the early stages of planning a class reunion. I decided I’m going to use a WordPress blog as the official planning site.

Right now, I need to collect information from classmates. Does anyone know of a nice plugin that would make this easy? Ideally, I’d like to just be able to put together a form for visitors to fill out. I’d like to be able to define the questions and then leave open fields for them to answer the questions. Then, the answers would be 1) emailed to me and 2) entered into the SQL database and show in the admin section.

Has anyone heard of a plugin like this? If not, would anyone be willing to create one for pay?
