a good opinion plugin or theme?

  • a client wants me to make a website were students can rate their accommodation where they life. so it is basically a really simple website.

    they fill in there email address,
    choose their accommodation,
    awnser basic questions,
    and sumbit.

    all the submissions need to be private, and saved so some one can look at them and help the students.

    who knows a plugin or a theme where I can achieve something like this?
