    _yoast_wpseo_primary_category is also assigned to posts with only one category set, correct? It seems to be unpredictably not assigning a primary category for me. Any ideas?

    The Primary category feature is used once per post. It is not automatic, you will need to manually assign it. Can you confirm you are able to manually click on a category and then mark it as “primary”? See: https://pasteboard.co/GLrWRJv.png

    You can only manually mark a post with a primary category if it has more than one category is selected. What I’m saying is that posts with only one category are seemingly arbitrarily set with a corresponding _yoast_wpseo_primary_category. What might be the cause of this?
