500 Internal Server Error

I installed the Apply Online plugin in both my Dev and Production environments. It seems to work fine in my Dev environment but in my Production enviroment I am getting “An unexpected error occured with error code: 500 Internal Server Error. Please contact us for more information.” I initally though it was an file attachment issue, but even without that field, I still get this error. The only differece from Dev and Production is production runs captcha. Any thoughts on how determine what is causing this issue?
