404 Page on "Log Out"

When clicking “Log Out” I receive a 404 page from my chosen theme/template. It seems to be logging me out of WP properly, because I have to log in again upon returning to the sign-on page. I’m wondering if the plugin just isn’t redirecting back to the proper URL?
The resulting page is: https://hostname/wp/
Which happens to be the WP_SITEURL variable for my site. But that’s just the location of my WP installation. It might be better to redirect to WP_HOME instead. In my case, that would be https://hostname/
