404 Errors

We’re trying to add an item to the BP menu and when I publish the item, it all seems to work perfectly.
However, when I click the link in the user profile to see the new tab I’ve added, I get a 404 error. This happens no matter what content I put in the tab, if I lock or unlock for groups, and if I’m logged into other users. Am I missing something in the setup? I don’t even get anything in the error log, just the 404 in the access logs. Thanks!
