404 Error on Activation

On initially loading this page, I see the Activate checkbox with the code from the URL in the box. Clicking Activate shows a blank white page. In the Network tab of my browser, it’s saying the page has a 404 error.
My .htaccess is barebones, just the default WP directives. Is there a setting somewhere I don’t have checked? Is there some other code I should be looking for?
